California Advancing and Innovating Medi-Cal (CalAIM)
California Advancing and Innovating Medi-Cal (CalAIM) is a statewide initiative designed to improve the way health care and supportive services are delivered to Medi-Cal members. Through CalAIM, individuals receive whole-person care that addresses physical, behavioral, and social needs to promote long-term wellness and stability.
How WestCare Supports CalAIM
As a trusted provider in California, WestCare partners with health plans, community organizations, and local stakeholders to deliver services under CalAIM, including:
- Enhanced Care Management (ECM): Intensive, hands-on support for individuals with complex needs, including care coordination, health system navigation, and advocacy.
- Community Supports (CS): Flexible, health-related services designed to address social drivers of health, such as:
- Housing Transition and Navigation Services
- Housing Tenancy and Sustaining Services
- Direct connections to other Community Supports such as medically tailored meals, recuperative care, respite services, etc. (as available through health plans)
Who Can Benefit?
CalAIM is designed to support Medi-Cal members who:
- Are experiencing homelessness or housing instability
- Live with serious mental illness or substance use disorders
- Transitioning from incarceration.
- Have frequent avoidable hospital or emergency department visits
- Struggle to manage complex medical and behavioral health needs
